New Patient Registration

Come to the surgery and ask for a new patient pack. You should inform reception if you have a preference to see a particular doctor, and this will be recorded in your notes.

Please provide proof of identity and address when returning the form. We are unable to book an appointment for you until we have received this.

All new patients are offered a consultation with the doctors. This is recommended to allow initial medical assessment and advice  (within three months of registration). 

Please note that this practice will not tolerate violent or abusive patients. Such patients will be removed from the practice list.
